Utah Tech University offers dental insurance through EMI Health Dental. Dental plan uses preferred providers but non-preferred provider coverage is available at the same rate as preferred with the employee paying any remaining balance.

To add, remove, or make any changes mid fiscal year, login to NEOED / eForms and submit an EMI Dental Change form.  Upload any necessary documents (e.g. birth certificate, marriage license, etc.)
